If you are a woman, 18 or older, living in the U.S. and have experienced:

  • Elevated LDL (“bad”) cholesterol and had angina, a heart attack or stroke OR
  • Elevated Lipoprotein Lp(a) and had angina, a heart attack or stroke OR
  • Cardiomyopathy (any form of cardiomyopathy)
